Aim. To evaluate the clinical effectiveness and safety of the Ross procedure in children with congenital aortic valve disease. The main aim of the study was to determine the physiological and hemodynamic advantages of the Ross operation compared to conventional valve replacement techniques. Materials and methods. A total of 14 pediatric patients who underwent the Ross procedure at the Medion Innovation Center between 2022 and 2025 were analyzed. The primary diagnoses were aortic stenosis (n = 9) and aortic insufficiency (n = 5). In all cases, the full-root replacement technique was applied. Postoperative echocardiographic parameters, including transvalvular pressure gradient and degree of regurgitation, were assessed. Results. A significant improvement in postoperative hemodynamic parameters was observed in all patients. The mean aortic pressure gradient decreased from 45 ± 12 mm Hg to 8 ± 3 mm Hg, and no clinically significant regurgitation was detected. No early postoperative complications or autograft dysfunction were reported during the follow-up period. Conclusion. The Ross procedure is an effective and safe surgical method for treating congenital aortic valve pathology in children. It restores near-physiological hemodynamics, improves long-term outcomes, and reduces the need for reoperation. The results support broader implementation of the Ross procedure in pediatric cardiac surgery practice.
