В статье представлен подробный анализ современных доказательных подходов к профилактике аллергических и псевдоаллергических реакций на местные анестетики в стоматологической практике. Несмотря на чрезвычайно низкую частоту истинной IgE- опосредованной аллергии, значительная часть нежелательных реакций ошибочно расценивается как «аллергия». В работе рассмотрены основные факторы риска, клинические алгоритмы, методы выбора безопасного анестетика, техника медленного введения и необходимость готовности к неотложной помощи. Включены две аналитические таблицы и одна диаграмма, отражающие стратификацию риска и этапы профилактического клинического управления. Представленные меры позволяют существенно повысить безопасность пациента и оптимизировать проведение анестезии в амбулаторных условиях.
