Acute infectious rhinosinusitis is a common clinical form of respiratory pathology in childhood. Despite the fact that the main etiological factor causing rhinosinusitis in 90% of cases are viral infections, 13% of them are complicated by the development of bacterial inflammation. Acute respiratory diseases observed in children with the appearance of persistently recurrent rhinosinusitis can lead to the formation of multifactorial chronic inflammatory diseases of the upper respiratory tract - allergic, non-allergic rhinitis and chronic rhinosinusitis. Despite the abundance of scientific data, the pathogenesis of acute infectious rhinosinusitis development is poorly understood. The article describes the problem of treatment of acute infectious rhinosinusitis in children from the point of view of modern views on the development of inflammation in the nasal mucosa, the role of innate immunity and the functioning of mucociliary clearance. Information about clinical manifestations of acute infectious rhinosinusitis is presented, clinical and instrumental diagnostic criteria are given. Mechanisms of action of nasal decongestants, peculiarities of their use, immunological and anti-inflammatory effect of drugs are described. Attention is focused on the mechanism of action of oxymetazoline. Recommendations on the use of oxymetazoline in paediatric practice are given.
