SOME FEATURES OF DAMAGE TO THE HEAD STRUCTURE DURING DIFFERENT TYPES OF INJURY FROM THE IMPACT OF DULL OBJECTS

The purpose of the study was to determine the features of the damage to the structure of the head, depending on the mechanogenesis of injury from the impact of blunt objects. Materials and research methods. The analysis of the results of a forensic medical examination of corpses associated with mechanical trauma from the impact of blunt objects, performed over the past 5 years in the Samarkand branch of the Republican Scientific and Practical Center for Emergencies of the Ministry of Health of the Republic of Uzbekistan, was carried out. The materials of the circumstances of the injury, the conclusions of the SME, medical records and the results of additional studies were studied. The obtained data are subjected to statistical processing. The results obtained: it was found that the leading place in the origin of TBI is occupied by a car injury (65.4%) and the impact of various blunt objects (19.6%). In 30.9% of the dead, TBI occurred without damage to bone structures, and in 69.1% of the dead, TBI occurred with fractures of the skull bones. Victims with TBI in 62.4% of cases died at the site of injury, in the rest - 37.6% of cases, the lethality of the victims occurred in the nearest medical and preventive institutions. In 20.6% of the victims, TBI occurred on the background of alcohol intoxication of varying degrees. Fractures of the bones of the skull in TBI in most cases (36.0%) covered all the bones of the skull with a transition to its base, which were accompanied by intrathecal hemorrhages, and sometimes traumatization of the brain structure. Conclusions: TBI was observed predominantly in males (79.0%), women accounted for 21.0%, in terms of age, people of the most working age predominated - 18-60 years old (70.2%). In falls and in all types of transport injuries, cases of TBI with fractures of the skull bones predominated. In injuries from the impact of various blunt objects or tools, caused by illegal actions, TBI with bone fractures and without lesions of the bone structures of the head, were noted to the same extent (63 and 64, respectively). Epi- and subdural hematomas in the volume from 70.0-80.0% cm3 to 80.0-160.0 cm3 were most often noted in the frontal-temporal-occipital part. Subarachnoid hemorrhages in most cases were common and in the areas of bruises - the most intense, hemorrhages in the brain substance covered the cortex and subcortical part of the white matter, and with multi-comminuted fractures in the areas of bruises, softening of the brain substance was also detected. In the early period of injury, the death of victims with TBI occurred due to severe contusion, edema and swelling of the brain, in the subsequent stages of the injury, infectious complications were detected - meningitis and meningoencephalitis. Combined injuries of the chest, abdomen and musculoskeletal system contributed to the onset of death.
