FEATURES OF THE COURSE, CONSEQUENCES AND SEVERITY OF INTRA-ARTICULAR FRACTURES FROM THE IMPACT OF BLUNT OBJECTS

The purpose identification of the nature, features of the course, consequences and criteria of forensic qualification of the severity of intra-articular fractures. Materials and research methods. The results of treatment of 400 patients with intra-articular fractures who were treated in the Samarkand branch of the Republican Specialized Scientific and Practical Medical Center of Traumatology and Orthopedics for 2021 were studied. The largest number of patients with intra-articular fractures were people of working age (18-60 years)- 74.8% and male persons predominate among the victims (58.7%). The results obtained: in various types of trauma from exposure, intra-articular fractures were most often noted in the ankle and hip joints (44.0 and 20.0%, respectively), then in the wrist (12.0%) and shoulder joints (11.5%). In the knee and elbow joints, such injuries were relatively less frequent (7.7% and 4.75%, respectively). Intra-articular fractures with impaired congruence of articular surfaces were most often formed (66.7%), followed by comminuted intra-articular fractures (28.8%). The smallest number (4.5%) were fractures with preservation of congruence of articular surfaces. The duration of limb immobilization in case of joint structure damage, depending on the nature of the injury, usually ranges from 5-6 to 9-10 weeks. In our observations, in most cases, regardless of the nature of fractures, the immobilization period corresponded to this period. Exceeding the indicated period of immobilization in some patients up to 9-10 weeks was due to the duration of fracture healing, and in some cases - belated appeals of patients to remove plaster splints and bandages. Conclusions: In the long-term period of trauma, complications in the form of ankylosis, aseptic necrosis and degenerative-dystrophic processes in the joints were noted in fractures with preservation and violation of the congruence of articular surfaces and, for the most part, in comminuted intra-articular fractures. Under these conditions, the duration of the health disorder was up to 5-6 months, and the volume of persistent loss of general disability ranged from 35 to 45%. Such conditions were classified as serious bodily injuries, and uncomplicated fractures were classified as moderate injuries. An intra-articular fracture of the head of the shoulder, as well as a fracture of the proximal femur and fractures of the ankles of both tibia were classified as life-threatening to serious bodily injury. It has been established that the direct mechanism of trauma prevails in the origin of intra-articular fractures.
