Giant right frontoparietal parasagittal meningioma with falx– sinus angle invasion, epileptic seizures, left-sided hemiparesis, and progressive mass effect: a case report

Parasagittal meningiomas involving the middle third of the superior sagittal sinus are among the most challenging intracranial tumors because of their close anatomical relationship with functionally important cortical veins, major venous drainage pathways, and motor cortical areas. This article presents a clinical case of a giant right frontoparietal parasagittal meningioma with falx–sinus angle invasion, epileptic seizures, left-sided hemiparesis, peritumoral brain edema, and a pronounced mass effect. A 31-year-old woman presented with progressive headache, dizziness, paresthesia in the left extremities, emotional lability, sleep disturbance, and recurrent epileptic seizures. Serial magnetic resonance imaging demonstrated progressive tumor enlargement from 60×55×48 mm in 2023 to 83×65×62 mm in 2026. The lesion was associated with displacement of the midline structures to the left by up to 17.2 mm. According to the Sindou– Alvernia classification, superior sagittal sinus involvement corresponded to type II. Documented tumor growth, progressive focal neurological deficit, persistent seizure disorder, peritumoral edema, and marked intracranial mass effect were considered clear indications for surgical treatment. Particular attention during surgical planning was given to preservation of superior sagittal sinus patency and functionally significant cortical venous drainage. This clinical observation emphasizes the importance of serial volumetric assessment, clinical-radiological correlation, and a personalized maximal safe resection strategy in the management of giant parasagittal meningiomas involving the falx–sinus angle.
