Despite 40 years of history, HIV continues to be a major global public health problem. An effective treatment for HIV infection and disease has claimed the lives of approximately 40.1 million people to date. By WHO statistics from the end of 2021, around 38.4 million people live with HIV worldwide, 650 000 of whom died from HIV-related causes and newly 1.5 million people became infected with HIV. The development of new therapeutic agents and treatments has made HIV manageable. However, this achievement cannot be called comforting, due to the presence of many circumstances that develop even with good adherence to treatment. One such important and life-threatening clinical condition is damage to nervous system. HIV-related neurological disorders affect 40% to 70% of infected people. They affect central nervous system (CNS) and peripheral nervous system (PNS) at any time during the course of infection, although most neurological disorders become apparent with severe immunosuppression in the AIDS phase. There are two main groups of neurological disorders associated with HIV-1 infection. One group is opportunistic infections of CNS and PNS that occur as a consequence of HIV-induced immunosuppression, the other group includes primary neurological syndromes caused by the HIV virus itself, including neurocognitive disorders and peripheral neuropathies. Opportunistic infections of the CNS that occur with HIV infection are the subject of this review and are a description of impact of these infections on the CNS, methods for their diagnosis and treatment.
