Diabetes mellitus and visual impairment: a modern approach to the problem, diagnosis, and preventive strategies

Objective: To assess how glycemic control (HbA1c) affects macular structure and retinal neurodegeneration. Methods: Prospective single-center study of 124 patients (247 eyes) with type 1 and 2 diabetes (2023–2024), stratified by HbA1c (<7.0%, 7.1–8.9%, ≥9.0%). All underwent ophthalmic exam and OCT (macula, RNFL, GCC). Results: Higher HbA1c was associated with progressive macular thickening and increased frequency of clinically significant macular edema (observed only at HbA1c ≥9%), together with RNFL/GCC thinning. Conclusion: Poor glycemic control correlates with maculopathy and retinal neurodegeneration; OCT enables early detection and monitoring.
