Relevance. Primary open-angle glaucoma (POAG) remains one of the leading causes of irreversible blindness worldwide. Purpose of the study. To evaluate the effectiveness of 3.5% sodium carboxymethylcellulose (Na-CMC) hydrogel compared to viscoelastic and standard trabeculectomy technique without adjunctive materials in the formation of the filtering bleb (FB) following antiglaucoma surgery in patients with primary open-angle glaucoma (POAG). Materials and methods. The study included 105 patients with stage II–III POAG who underwent trabeculectomy. Patients were divided into three groups: main group 1 (Na-CMC), main group 2 (viscoelastic), and a control group (no adjunctive materials). Morphological evaluation of the FB was performed using the Indiana Bleb Appearance Grading Scale (IBAGS) at 1, 3, 6 months, and 1 year postoperatively. Results and conclusion. At three months, the control group showed a decrease in FB height and width, along with increased vascularization, in contrast to the Na-CMC and viscoelastic groups. In main group 1, FB parameters were significantly higher and vascularization significantly lower compared to the control group (p<0.001). These differences persisted at the 1-year mark and were most pronounced in the Na-CMC group. The Seidel test remained negative across all time points, indicating stable bleb integrity without leakage. The use of Na-CMC hydrogel contributes to more stable formation of the filtering bleb and may provide a sustained hypotensive effect in POAG patients undergoing surgical treatment.
