Stratification: Syringomyelia – surgery for cyst >4 mm/progression; conservative for stable forms. Friedreich's ataxia – predominantly conservative; surgery for scoliosis/stenosis. AVM – embolization/resection for bleeding risk; observation for asymptomatic cases. A treatment stratification based on pathology specificity and POD risk is proposed. A multidisciplinary approach optimizes outcomes.
