Increasing the effectiveness of the rehabilitation of patients after a stroke remains one of the most important strategic tasks of Russian healthcare. Cognitive impairments that develop in the early period after a stroke have a negative effect on the rehabilitation process. Their timely detection and correction can improve the functional outcome. At present, the use of many drugs with neuroprotective properties does not have a serious evidence base. A search is actively under way for the most effective rehabilitation regimens including modern medicines. The article provides a review of recent works devoted to the possibilities of using the drug memantine in ischemic stroke. The own experience of two institutions allows us to speak of an improvement in cognitive functions and an increase in motivation while taking it, which opens certain prospects for its use in the prolonged rehabilitation of patients with cognitive impairments in the early rehabilitation period after a stroke.
