Post-stroke epilepsy is one of the most significant complications of stroke, exerting a negative impact on the recovery of cognitive and motor functions as well as on patients’ quality of life. Despite the accumulated clinical evidence, the issues of timely diagnosis, predictors of development, and the choice of optimal therapy remain controversial. Current research is focused on the role of neuroinflammation, blood–brain barrier disruption, post-ischemic neuroplasticity, and molecular biomarkers in epileptogenesis, which suggests that this condition should be considered not only as a complication but also as a potential target for early intervention.
