Androgenetic alopecia (AGA, ICD-10: L64) is the most common form of non-scarring alopecia worldwide, significantly impacting quality of life. Despite advances in understanding its pathogenesis, standard treatments (topical/oral minoxidil and 5α-reductase inhibitors) show limited efficacy in 30–40% of patients and do not address the full spectrum of contributing factors. This review synthesizes current data on the multifactorial pathogenesis of AGA, emphasizing the interplay of androgen-dependent mechanisms with immunogenetic factors (e.g., MTHFR, VEGF, KGF polymorphisms), biochemical markers (homocysteine, folate, vitamin B12), chronic microinflammation (IL-1, VEGF), and microvascular disturbances. Particular attention is given to the shift toward personalized, precision-medicine approaches incorporating trichoscopy, Doppler imaging, and biomarker profiling. Gaps in regional data, especially in Central Asian populations, justify comprehensive studies aimed at optimizing diagnostic algorithms and complex therapeutic protocols. Such integration is expected to improve treatment outcomes, reduce progression, and minimize side effects.
