The article discusses the method of surgical treatment of patients with complicated forms of cholelithiasis, using hybrid minimally invasive technologies (laparoscopic and endoscopic methods in an X-ray surgical operating room). Between 2017 and 2023, 1163 patients with acute cholecystitis were operated on. Of these, 107 (9.2%) patients had a combination of acute cholecystitis with the presence of stones in the extrahepatic bile ducts with symptoms of obstructive jaundice and cholangitis. In this group, we used hybrid minimally invasive technologies for sanitation of the bile ducts in 55 patients. The conversion rate to open access was 7.2% (4 patients), postoperative complications in the early postoperative period were noted in 5.45% (3) of patients. No postoperative mortality was observed. Hybrid minimally invasive operations on the gallbladder and bile ducts for complicated forms of cholelithiasis are a low-traumatic intervention with a minimum number of postoperative complications. In the absence of contraindications, this technology can be widely used in institutions providing emergency surgical care.
