Fibrocystic mastopathy developing in the setting of ovarian dysfunction is a heterogeneous hormone-dependent condition whose clinical course is largely determined by disturbances in hypothalamic-pituitary-ovarian regulation. Luteal phase insufficiency, anovulatory cycles, relative estrogen excess, progesterone deficiency, and abnormalities in prolactin secretion stimulate proliferative processes in breast tissue and may reduce the effectiveness of standard unified treatment regimens. The aim of this study was to evaluate the effectiveness of a differentiated approach to the treatment of fibrocystic mastopathy in patients with ovarian dysfunction, taking into account their individual hormonal characteristics. The therapeutic strategy is based on a comprehensive assessment of the clinical condition, reproductive and menstrual status, serum levels of estradiol, progesterone, prolactin, follicle-stimulating hormone (FSH), and luteinizing hormone (LH), as well as findings obtained by ultrasonography, sonoelastography, and mammography. The choice of treatment strategy should be differentiated and should include correction of the identified hormonal imbalance and personalization of therapeutic measures according to the type of ovarian dysfunction and the structural changes observed in the mammary glands. Treatment effectiveness is assessed by a reduction in pain intensity, regression of diffuse and cystic changes, normalization of the hormonal profile, and slowing of pathological progression. The use of a personalized approach makes it possible to account for the pathogenetic heterogeneity of the disease, thereby improving the clinical effectiveness of therapy, reducing the recurrence rate, and facilitating the development of an optimal long-term follow-up system for women with combined breast and ovarian pathology.
