The article is devoted to a comprehensive assessment of the therapeutic and diagnostic tactics for children with perinatal CNS damage at early stages. The aim of the study is to examine the dynamics of the factors of perinatal brain damage in order to predict the level of neurological disorders and to optimize treatment tactics at early stages so as to prevent disabling complications. Material and methods of the study. The studies were carried out on children in the period from 2022 to 2024 on the basis of the Multidisciplinary Clinic of Samarkand State Medical University (MC SamSMU), in the departments of the maternity complex, the department of neonatal pathology, pediatric intensive care (for newborns), the department of pediatric neurology and the outpatient department (at MC SamSMU). Twenty mothers (from the main group) were observed throughout the entire period of pregnancy; all mothers of the main group filled in a questionnaire whose task was to study the history of somatic status before pregnancy, the obstetric and gynecological status during pregnancy and the outcome of delivery. The control group consisted of 20 healthy children of identical age (1-2 years) and sex. Results: the health status of the mothers during pregnancy and childbirth forms certain functional and organic disorders in the children. Accordingly, the analysis of extragenital changes and of the course of the ante- and intranatal period in the mothers is undoubtedly indicative in the examined children. Thus, the analysis showed that among the complications of pregnancy in the mothers of the examined children there are many aggravating factors, such as placental insufficiency, the pathomechanism of the role of antenatal activation of autoimmune processes and chronic intrauterine fetal hypoxia, which directly affect the development of neurological deficit in children. Conclusions: thus, the proposed scheme for optimizing complex therapy (cerebrolysin, sermion) indicates a high degree of treatment efficacy, in which clinical improvement of the residual-organic type of cerebral changes was observed; at the same time the choice of treatment tactics made it possible to increase accuracy and accelerate the detection of perinatal CNS damage in newborns, to reduce the risk of various complications and the level of disability in children.
