BARIATRIC SURGERY OPTIONS FOR PATIENTS WITH SEVERE OBESITY AND CONCOMITANT HIATAL HERNIA

Annotatsiya

The aim of this literature review was to analyze current evidence on bariatric surgery options for patients with concurrent severe obesity and hiatal hernia. A comprehensive search of electronic databases including PubMed, Scopus, Web of Science, and Embase (2010–2024) identified 48 relevant studies reporting outcomes of simultaneous bariatric procedures and hiatal hernia repair. The results demonstrated that Roux-en-Y gastric bypass provides superior reflux control and lower hernia recurrence rates (2–8%), making it the preferred option for patients with symptomatic hernias. Sleeve gastrectomy with concurrent repair also showed favorable outcomes, serving as a viable alternative with slightly higher recurrence rates (4–15%). Gastric banding is generally discouraged due to complications, while combined procedures overall maintained acceptable safety profiles comparable to isolated surgeries. The findings highlight the importance of individualized surgical decision-making based on specific hernia characteristics and patient comorbidities to ensure optimal long-term outcomes.
