Odontogenic periostitis is one of the most common inflammatory diseases of the maxillofacial region and is particularly relevant in elderly patients. Age-related changes in the immune system, the presence of comorbid conditions, and a high degree of polymorbidity significantly complicate the clinical course and require a revision of standard treatment approaches. This review article analyzes current therapeutic strategies for odontogenic periostitis with regard to geriatric-specific features, highlights the principles of personalized care, emphasizes the importance of interdisciplinary collaboration, and discusses promising research directions, including the development of adapted clinical protocols, and the use of innovative technologies.
