PREMATURE BIRTH: OBSTETRIC RISK FACTORS AND THEIR IMPORTANCE FOR THE NEONATAL PERIOD

To assess the prevalence of preterm birth and to identify risk factors associated with extremely early, early, and late preterm deliveries. Materials and Methods. A retrospective study was conducted involving 11,500 pregnant women and their newborns who delivered at a level III maternity facility between 2019 and 2021. Data were obtained from the obstetric inpatient database. Twenty maternal, fetal, and obstetric factors were analyzed, including gestational age, birth weight, fetal sex, Apgar score at 5 minutes, maternal age (<20 years, 20–35 years, ≥35 years), parity, active smoking, pregnancy-related diseases, pregnancy complications (placental abruption, placenta previa, antepartum hemorrhage, polyhydramnios), fetal distress, and fetal growth restriction.Results. The analysis showed that significant risk factors for preterm birth included placental abruption, placenta previa, cervical length <25 mm, intrahepatic cholestasis of pregnancy, hypertensive disorders, chorioamnionitis, anemia, maternal age <20 or ≥35 years, primiparity, active smoking, and fetal distress. These factors differed significantly from those observed in term deliveries. Conclusion. The overall prevalence of preterm birth was 8.4%. Extreme and early preterm births were mainly associated with pregnancy complications, while intrahepatic cholestasis of pregnancy was a predictor of late preterm birth. Different subgroups of preterm infants demonstrated distinct risk profiles, indicating the need for differentiated clinical management strategies.
